An important part of filling in an outcome map is adding sources, success criteria and evidence to stepping stones within your pathways. These terms are briefly defined below:
Sources are where your evidence originates from. For example, you may create a source called "Client feedback forms".
Evidence consists of individual files or links that contain or point to the data you are using. Multiple evidence files can be added within a source; you can think of this like adding files to a folder on your computer. For example, you may upload several different Word documents containing client feedback within the "Client feedback forms" source.
Success criteria are criteria that you define that will tell you when you are succeeding in a certain area. For example, you may create a success criteria called "Clients tell us they feel welcome and supported when accessing our services".
Sources, success criteria and evidence can be added to your outcome map in a few different ways.
If you need to add these elements to several different stepping stones within a pathway, it may be easiest to use the Evaluation Manager, which gives you an overview of all your evidence for a specific pathway. The Evaluation Manager can be accessed by going to the Tracking section of OutNav, and clicking the 'Evaluation Manager' button at the top right of one of the pathway you want to work on.
To add sources, success criteria and evidence to an individual stepping stone, you can use the Update Analysis page, which is accessed by clicking a stepping stone within a pathway in the Tracking section of OutNav.
Once you are on the Evaluation Manager or Update Analysis page, follow the steps below to add sources, success criteria and evidence.
Adding a source
- Click on the 'add source' button to add the source to the stepping stone
- Choose from an existing source (click on the 'Select Source' text) or add a new source
- Add your plans for collection (if any)
- Submit
Adding evidence
- Once you have added a source, you can attach relevant evidence by clicking on the add evidence button
- Choose whether you want to upload a file or add an online link
- Submit
(Please note that evidence files in excess of the file size limit may not be uploaded directly to OutNav - instead we recommend hosting these elsewhere and linking to them.)
Adding success criteria
- Click on the 'add success criteria' button
- Type in your success criteria
- Submit
Editing and deleting sources, success criteria and evidence
To view, download edit or delete your source, evidence or success criteria, hover over the item you want to change and use the following icons:
